COVID Vaccination
All 4 practices of our network are committed to provide COVID-19 vaccination to not only our patients, but patients from outside our network.
Social Prescribing
Social Prescribing is an approach that connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.
North Trafford PCN Social Prescribing service therefore offers time to focus on ‘what matters to you’ as an individual, and to enable an action plan to be co-produced together.
Social prescribing works particularly well for patients who:
·have one or more long term conditions
·need help to make positive lifestyle changes including regular exercise
·who need support with low level mental health issues
·who are lonely or isolated
·who have complex social needs which affect their wellbeing.

If you are an adult patient at any of our member practices, and need support in the areas mentioned above, please get in touch with your GP so you can be referred to our services.
